Blood Bank
Blood Products Usage
FWB - Fresh Whole Blood
*   This product is indicated for patients
    requiring massive transfusions due to
blood loss.  They need clotting factors
in addition to RBC's due to loss of both.
*   Cases of moderate hemorrhage are more
   appropriately treated with crystalloids/
   colloids and PRBC's.

PRBC's - Packed Red Blood Cells
*   Indicated for patients symptomatic from
   moderate anemia.
*   Does not provide clotting factors.
*   Use in combination with colloids/cystalloids
if FWB is indicated and it is
   not available.

FFP - Fresh Frozen Plasma (Life = 1 yr)
*   Indicated for deficiencies of clotting
factors (warfarin toxicity, DIC, severe hepatic
   disease.... Hemophilia A, von
   Willebrand's disease).

FP - Frozen Plasma (Life = 5 yrs)
*   Contains Vitamin K dependent clotting
   factors (II,VII,IX,X).
Use as a source of albumin in certain
   cases (parvo, burns).
*   If protein loss is via renal or GI routes
then  FP will be lost also.  Use colloids
   in these cases.
*   It takes 22.5 ml/kg of plasma to raise
albumin 0.5g/dl, Therefore in most cases
plasma use for hypoalbuinemia is futile.

Transfusion Reactions
Immunologic - clinical signs include hemolysis, urticaria, anaphylaxis, pyrexia, tachycardia, vomiting and shock.  Transfusion should be stopped and saline, antihistamines and corticosteroids given.

Nonimmunologic - include hypocalcemia from citrate toxicity, infectious disease transmission and sepsis due to bacterial contamination of blood products.  Signs include pyrexia, tremors, seizures, tachycardia, dyspnea and vomiting.  Blood cultures of the patient and cultures of the transfusion unit should be performed and aggressive, supportive care started.

Crossmatch Your Patients
